If you are inquiring about a custom design, please do not use the form, INSTEAD please copy and paste the following into an e-mail providing the information indicated and a file containing the image to be used.  Contact Joan  Type of item to be embroidered, Location where embroidery is to placed, and additional lettering and/or monograms to be applied.
